1. Brief Summary of Care
This allows our medical team to review your history, diagnoses, and medications to assess eligibility.
Requirements:
- Must be dated within the last 6 months
- Must include diagnosis and current medications
How to get it:
Contact your GP surgery and request a Brief Summary of Care Record. This can usually be requested by email, through your GP portal, or in person. An electronic PDF document is the preferred format as the document file limit is 8MB.
2. Verification
Proof of Address
Confirms the address where medication would be delivered:
- Utility bill or Bank statement
- Driving licence
- Official government letter
Photo ID
Official identification to confirm your identity and date of birth:
- Passport
- Driving licence
- Government-issued photo ID
3. Your Privacy
All documents are stored using secure, GDPR-compliant systems and are only accessed by authorised members of the medical team.
These documents allow our team to determine if you are eligible for a formal consultation.
